Quick answer: Steffy Forrester and Carter Walton slept together on the Aug. 17 episode, Steffy is now pregnant with his child, and as of the Oct. 9 episode Finn still hasn't been told. Carter's answer to the problem is to pass the pregnancy off as surrogacy for him and Daphne.

What happened between Steffy and Carter?

On the Aug. 17 episode, Steffy and Carter were drinking together at Forrester Creations and slept together. The morning after played out on Aug. 18: they woke on the office couch, and the regret arrived before either of them was fully sober.

Both had reason to think they were free. Steffy believed her marriage to Dr. John "Finn" Finnegan was finished over what happened in Monaco involving Sheila. Carter thought Daphne, his wife, had left him for good.

Then both spouses came back. The night only made sense under an assumption that stopped being true by breakfast.

Is Steffy pregnant, and is the baby Carter's?

Yes to both, and Carter has known since mid-September. Steffy told him she had taken two positive tests, and that the child is his, not Finn's.

Her instinct was to tell Finn immediately. Carter's was the opposite — he argued that the truth would take apart four lives at once, and asked her to keep both the pregnancy and the paternity quiet.

What is Carter's surrogate plan?

Daphne cannot carry a child after a fertility diagnosis earlier this year, and that has been the wound in their marriage since she came back. Carter's fix is to have Steffy present herself as egg donor and surrogate for the two of them — so the baby he fathered arrives as a gift rather than a betrayal.

Steffy shot it down on Wednesday, Sept. 16. She's already pregnant, she can't pretend otherwise for nine months, and by her own account the guilt is eating at her.

So on Friday, Sept. 18, Carter stopped asking. Daphne walked into the cliff house and he announced that Steffy had already said yes, then turned to her and told her to confirm it. The episode ended on Steffy's face, not her answer.

What happens the week of September 21?

The soap press previewed Steffy caving on Monday, Sept. 21 and giving Daphne the promise. As of the Sept. 30 episode she had not clearly agreed: Carter was pressing her, insisting Daphne already believes she has said yes. On Oct. 1, Daphne told Ridge and Brooke that she and Carter had a surrogate in mind, without naming Steffy (per Soaps.com's recap), while Steffy privately knows the surrogacy story is a cover. Finn was out of town on Sept. 30 and did not know about the pregnancy or the paternity.
